SERVICE POLICIES + PRICING
Payment is due upon receipt of services. We accept cash, and major credit cards (MasterCard, VISA, Discover, American Express and debit).
We are currently in-network with Oxford and Blue Cross Blue Shield health insurance plans and out-of-network with United Healthcare. We can provide you with a superbill to submit to other insurance companies.
Though walk-ins are welcome, visits by appointment are preferred to insure that our patients receive individualized attention and care. If it is necessary to cancel an appointment, please let us know at least 24 hours in advance so other patients may be offered the availability. If an appointment is missed or cancelled less than 24 hours in advance, a charge will be applied.
For All Non-Medical Appointments:
When scheduling non medical appointments you will be required to provide a credit card number to HOLD your appointment(s) as we do have a 24 hour cancellation policy. This policy states that you agree to call the office directly if you need to make any changes or cancel within 24 hours of the appointment time. If you failed to cancel within 24 hrs, your credit card will be charged a $35 fee. In the event of same day cancellation, or missed appointment, the credit card on file will be charged 100% of the service fee. So PLEASE be sure to select a time that works with your schedule and if anything comes up just give us 24 hours and we will happily reschedule your appointment.
For an additional fee, we will travel to locations within the Tri-State area. All home visit appointments require a credit card to hold the appointment. Cancellations must be made at least 24 hours in advance. Prices will vary depending on location. Please call for exact rates.
